A "Near Miss" is an unplanned event that has the potential to cause harm, but doesn't actually results in harm, injury, environmental and equipment damage, or any interruption in normal operation.
A near miss can take place due to an multiple reasons for example;
- Mismanagement.
- Poor communication.
- Training shortcomings.
- Equipment and tools misuse.
- Incompetent people on job.
- Organizational Factors.
Examples of Near Miss
- Falling objects landing near working people.
- A worker tripping over a broken tile but holds a nearby objects to steady himself.
- A worker trips over the step and doesn't get any injury.
- Excavation of trench without identifying and isolating the utility services underground.
- Crane operator nearly hit a worker.
