Document Review Form is used to prepare a summary of the expected changes that are to be done in the document, form, checklist, manuals etc. The competent person prepares the document, writes down the changes and forwards it to the HoD, MD or CEO for the changes in the document.
The document is changed, if there is any loophole in the procedure, any new activity or equipment is introduced or removed from the process, an incident has taken place and procedure needs to be changed.
