Accidents are rare still they do occur at the workplace endangering the lives of the workers and leaving an impact on the environment adversely. Furthermore, nobody will accept that his/her beloved one goes to work and gets injured or die there due to negligence. The regulation also requires the employer to ensure that the workplace is safe and if there is any accident, it is reported and investigated.

A formal incident investigation procedure should be devised and implemented. An Accident Investigation team should be formed comprising of competent people capable of conducting the incident investigation. They should be provided with the necessary training, information, tools, and resources so that they can conduct the training.
